POWERCHART ONCOLOGY GO LIVE

PAEDIATRICS ONCOLOGY

 

As part of the PowerChart Oncology (PCO) project, Clinical Documentation and CPOE Chemotherapy ordering will go live in Paediatrics on B6 inpatients at VH and within paediatric ambulatory care in the PMDU on April 4 at 7am. Nursing intake and many elements of the bedside nursing record for all patients registered on B6 & in PMDU will now be completed online. Chemotherapy orders will be done electronically.

ADULT ONCOLOGY

 

Adult Oncology will be going live May 2, 2016 with Clinical Documentation and CPOE Chemotherapy for patients registered to LRCP and C7. This also includes patients receiving Oncology treatments at St. Thomas, Woodstock and Wingham Hospital satellite Chemotherapy units.

Important note:

After a go live the oncology paper forms will no longer be available at the bedside. This information will be accessible to anyone who has access to PowerChart, FirstNet or SurgiNet and the patient’s online chart.

     

HOME OXYGEN - VH, UH and LRCP ONLY

 

Based on mandated changes from the Ministry of Health, hard copy, signed prescriptions are now required for patients who need home oxygen. In order to facilitate this new requirement enacted on March 1st, a new plan replaced the original single order.

QUICK ORDERS (OLD) BEING REMOVED FROM POWERCHART   As of April 19, 2016 the Quick Orders (Old) will be removed from the Menu within PowerChart. Quick Orders (New) is updated and maintained and can be found on the Menu. Please ensure that any Favorite folders are saved to your Quick Orders (New).

SLEEP LAB STUDIES DONE AT LHSC CAN BE FOUND IN THE CARDIORESPIRATORY INVESTIGATIONS FOLDER

 

After May 9 (new date), sleep studies done at LHSC will post the results under Clinical Documents/Reports on the Menu, in the Cardiorespiratory Investigations folder, which is found in the Medical Imaging folder.

PERIOPERATIVE TRACKING ICON ADDED IN POWERCHART - SURGEONS AND ANESTHESIOLOGISTS ONLY

 

In order to provide surgeons and anesthesiologists with access to the Perioperative Tracking boards, the Periop Tracking icon has been added to the PowerChart Navigation toolbar.
Access to specific tracking boards will be provided to surgeons and anesthesiologists on an individual level.

NEW STANDARD ADULT TPN ORDER - LHSC ONLY

 

There have been changes made to TPN orders. The new order can be ordered as a single order or PowerPlan. The new TPN Adult - Fluid Standard order PowerForm allows for multivitamin and trace elements to be added. Use the Discontinue (Future) TPN Standard order to discontinue the order.

SELF-MEDICATION PROGRAM (SMP) - LHSC ONLY   There is now a formalized process and policy in place to support the Self-Medication Program (SMP). This will affect providers, nursing and pharmacy.

NUCLEAR MEDICINE DOCUMENTATION - LHSC ONLY  

Nuclear Medicine will begin documenting the use of therapeutic radioisotopes on the MAR--these therapeutic radioisotopes will be viewable once administered as part of the MAR and MAR Summary.

Accessed through PowerChart, FirstNet and SurgiNet, LearnNow provides in the moment access to learning tools based on your role. Quick guides and videos are suggested based on your most recent actions in the EHR or can be searched for using key words. On March 30 LearnNow went live for a pilot group of Oncology staff and providers. See our website for more information.

PROBLEM OPENING PACS IMAGES ON IPADS, IBOOKS AND MACS/ANDROIDS HAS BEEN FIXED  

Changes have been made in PROD to enable user credentials to be passed through from PowerChart to Centricity seamlessly.
Providers who are currently logging into Centricity on their devices will no longer have to when viewing their patient’s images.
This will also allow providers who use MACs/Androids to access their patients’ PACS images on their devices.

This electronic process replaces the current paper process in which the healthcare professional completes a paper invoice, has the patient sign it, and then sends the copies to the appropriate departments.

PROPOSED ORDERS FROM PHAMACIST  

Description:

All Pharmacists have been given the ability to write proposed medication orders to providers. This access will be in addition to the existing Suggest Order used today.


What is the difference? Proposed orders activate immediately when accepted by the provider. A suggested order typically comes in a PowerPlan which is reviewed and then entered by the provider.

Description:

Currently when a Facility to Facility Discharge is completed it sits as a planned admission medication reconciliation on the patient's chart.  These planned Admission Medication Reconciliations are never removed if they are not used.

In order to remove clutter and to limit the potential of these old orders being activated on an encounter 3 months down the road, they be removed 21 days after being planned if they have not been activated.

Description:

ED clerical staff will be able to right-click and complete the Consult To Physician order on the Orders page when the consulting service arrives to see the patient. This will trigger the consult icon on the FirstNet tracking board to change to Complete and communicate to all staff that the consulting service has arrived and assessed the patient.

NEW CORPORATE PASSWORD STANDARD   Beginning January 20, when you are next prompted to change your corporate password a new standard will be required. When the password is changed the matching Cerner ID will be linked to the corporate ID.

POWERPLANS   Description:
  • PowerPlans will be updated with approved changes on an ongoing monthly basis on the third Tuesday of the month.  Exceptions to this will be changes that are related to an immediate patient safety issue.

BUILD CORRECTIONAL DOSE INSULINS BY BRAND NAME   Description:
  • When ordering insulin as in hospital medications, you can now search for correctional doses by brand name. For example, Humalog, NovoRpaid, and Apidra.
     
EXTEND DUPLICATE CHECKING TIME FROM 30 TO 120 MIN ON LAB TESTS   Description:
  • The system will look 120 minutes in the past and future to see if this is a duplicate request.
  • If there is a duplication an alert will pop-up to notify the ordering clinician, asking if there is a need to proceed or if the order currently being placed can be canceled in light of the duplication.
  • This will decrease the amount of tests being collected by Clinicians and processed by the labs, but we will also decrease the number of picks a patient must experience in their visit.
     
REQUEST TO CHANGE UNIT OF MEASURE FROM mEq TO mmol FOR ALL INJECTABLE SODIUM BICARBONATE ORDERS   Description:
  • Units of measure in all injectable sodium bicarbonate orders and PowerPlans will now be in mmol (sodium bicarb 1 mmol = 1mEq).

QUICK ORDERS   The Quick Orders page is being updated to improve on the speed of the system and decrease the amount of space it takes on the screen. You will need to adjust the tabs so that your department's Quick Orders tab is first on the list when you login and move your Favorites folders to the updated page if you use them. We will keep both the new and the old Quick Orders pages for one month to allow for transition time with the old pages being removed on January 12th.

POWERPLANS
  This video demonstrates how to order, add subsequent orders, initiate and complete a recurring Multi-Phase PowerPlan.

BLOOD PRODUCT ORDER CHANGES  

The job aid explains how Product Only PowerPlans for blood products are being removed and replaced with single orders for blood products that can be added to a PowerPlan or be proposed. It also includes instructions to remove the old PowerPlan from your Favorites.

The job aid explains that when a decision is made to not complete a blood product order the provider needs to order the Discontinue Blood Product order to notify both the Blood Transfusion Lab (BTL) and nursing.

MEDICATION RECONCILIATION
 
Providers now have the ability to plan admission medication reconciliations.  There have also been some patient-focused improvements to the discharge medication reconciliation process.

FACILITY TO FACILITY DISCHARGE
  This new functionality allows for orders to be copied from one encounter to another. This allows the provider at the sending facility to recommend a medication regimen on discharge and the receiving provider to accept or modify on the arrival to the receiving facility.

PROPOSE POWERPLANS
  A provider student (medical, nurse practitioner, midwife and dentist) can now propose PowerPlans to a reviewing provider who can then either accept, accept with modify, or reject the proposed plan.
